PCHC recognized by Neighborhood Health Plan of RI for care to Medicaid patients
Providence Community Health Centers has again been recognized by Neighborhood Health Plan of Rhode Island as a top provider of care to their Medicaid patients. PCHC achieved the highest performance for 13 measures in Neighborhood’s network of medical practices. The results, which were for 2023, were measured by the Healthcare Effectiveness Data and Information Set (HEDIS), one of health care’s most widely-used standardized quality performance measures, used across six domains of care.
“The delivery of quality care is a promise our entire team works to fulfill on a daily basis; it is great to have data that confirms we are performing at a high level for our Medicaid patients,” said Dr. Margaret Miller, Medical Director of Primary Care Services at PCHC. “Our patient care teams including our nurse case managers, integrated behavioral health teams, patient navigators, clinical informatics team and so many others work collaboratively to provide high level health services to Medicaid patients and everyone else who comes to us for care.”
PCHC ranked in the Quality Compass® Medicaid 90th percentile benchmark for a total of 18 HEDIS measures and achieved the highest performance in Neighborhood’s network on the following 13 measures:
• Follow Up After Emergency Department Visits for Alcohol and Other Drugs Dependence - 30 Days
• Follow Up After Emergency Department Visits for Mental Illness (Within 7 Days, Within 30 Days)
• Follow-Up Care for Children Prescribed Attention-Deficit/Hyperactivity Disorder Medication – (Continuation and Maintenance)
• Childhood Immunizations Status – (Combo 10 , Combo 3)
• Well-Child Visits in the First 30 Months of Life (First 15 months, 15 Months – 30 Months)
• Lead Screening in Children
• Prenatal and Postpartum Care (Postpartum Care, Timeliness of Prenatal Care)
• Weight Assessment and Counseling for Nutrition and Physical Activity for Children & Adolescent - BMI Percentile
• Eye Exam for Patients With Diabetes
• Hemoglobin A1c Control for Patients With Diabetes (HbA1c Control (9%))
• Blood Pressure Control for Patients With Diabetes
• Risk of Continued Opioid Use – 30 Days
• Use of Opioids at High Dosage
